Mission Statement To advance the scientific and medical research related to the diagnosis, treatment, cure and prevention of pancreatic cancer.

This year, 45,000 people will receive the devastating diagnosis of pancreatic cancer. Among them will be brothers, sisters, moms, dads, aunts, uncles, grandmothers and grandfathers. We stand together with you in the fight against this disease. We know that with more research, there is hope that we will find a cure. That’s why “Hope” is the theme of The Lustgarten Foundation’s 2012 annual report. Hope embodies the Foundation’s commitment to advancing pancreatic cancer research and to finding treatment options that move beyond what is possible today. Hope inspires our scientists and researchers. Hope also motivates and emboldens those who face pancreatic cancer to persist in the battle against it. The following pages will lead you through the Foundation’s journey in 2012, highlighting promising research results that are now moving into clinical trials. The Lustgarten Foundation continues to provide grants for innovative research projects that empower scientists to study enterprising new ideas that will lead to better diagnostics, treatments and improved patient outcomes. Our grant program has a broad reach, drawing proposals from the most talented and progressive scientists across the country. On page 8, for example, you will learn more about promising discoveries as well as the encouraging research trends we continue to watch closely. We are pleased about the progress of The Lustgarten Foundation Pancreatic Cancer Research Laboratory, which resides at the world renowned Cold Spring Harbor Laboratory on Long Island and focuses exclusively on pancreatic cancer research. On page 5 you can read about Dr. David Tuveson, M.D., Ph.D., who serves as the Foundation’s Director of Research and Director of the Lab, and learn more about our initial studies, which are centered on early detection, drug development and drug delivery. The Lustgarten Foundation committed $24 million to multi-year grants in 2012, which was the most research funding allocated by the Foundation in a single year. The Foundation’s total research contributions since its inception totals more than $65 million, and today, The Lustgarten Foundation is the nation’s largest private funder of pancreatic cancer research. Due to Cablevision’s underwriting of our administrative expenses, 100% of all funds raised go directly to this important research.

scott nelson: a 10 year survivor & mentor

Meet Scott. He was diagnosed ten years ago, at 49, with pancreatic cancer. Today, he is cancer free. At the time of his diagnosis, he was active, his career was in full swing and he was raising a family. One day, everything was wonderful, and the next day…he was told he had the worst cancer you could get. Scott was diagnosed early due to luck and a proactive primary care physician. He was having some gastrointestinal issues and went to see his doctor. The doctor’s office was in a hospital facility. He directed Scott to get an ultrasound to rule out abnormalities and they found a tumor. After many more tests, he was diagnosed and a treatment plan was formulated. The news was, at first, devastating, and the prognosis was worse. He thought of his children, his family and their eventual life without him. He wanted to see his children graduate college, get married, and have children of their own. Scott needed to find the right resources and do the very best he could to survive for his family. The tumor was inoperable due to its size and location. Surgery is the best option for curing pancreatic cancer. After 12 weeks of radiation and chemotherapy, Scott was eligible to have the surgery to remove the tumor. He then had 6 weeks of chemotherapy to keep it from coming back. Scott’s story is unique. Tragically, it does not reflect the statistics. But what Scott has learned is there is an urgent need for cutting-edge medical research for pancreatic cancer. “Research is crucial. It is the only real tool that provides hope for people who are already undergoing treatment, those who are newly diagnosed, and those who are yet to be diagnosed. Research means hope for the families of patients who are desperately trying to find the proper diagnosis and treatments to save their loved one” says Scott. He knows that not everyone will share his fortunate outcome. But he offered his story to inspire determination and hope. In this battle, both are essential. Scott says, “I support the work of the Lustgarten Foundation because hope lies in research. Hope lies in discovery. Hope lies in the dogged pursuit of new diagnostic tools and therapies that will someday lead to a cure. This is precisely what The Lustgarten Foundation does. They fund such breakthrough research and they support the urgent need for more clinical trials. Their unyielding hope and leadership in the field absolutely... inspires me.”

The Lustgarten Foundation Pancreatic Cancer Research Laboratory

The Foundation’s Research Map lays out a dynamic plan to focus on new research areas, and covers the entire range of issues associated with pancreatic cancer. The goal of the Foundation is to fund research that offers hope to patients at all stages of disease — while, ultimately, finding a cure. The three pillars of The Lustgarten Foundation’s Research Map are:

Understanding the basic properties of pancreatic cancer; how, when and what causes it to develop.

Developing ways to detect and diagnose pancreatic cancer early.



Generating new and better therapies and treatments.

Today, The Lustgarten Foundation currently has a total of 41 on-going research projects, including 7 clinical trials. The Foundation awarded a record number of funding grants in 2012 — including $24 million in multi-year grants to top-notch researchers and institutions around the world — bringing our total investment in research to $65 million. Thus, we maintain our unique position as the nation’s largest private funder of pancreatic cancer research.

Led by Dr. David Tuveson, The Lustgarten Foundation Pancreatic Cancer Research Laboratory is fully operational and supported by a team that now includes 14 scientists. The laboratory has been tackling pancreatic cancer on three different fronts: studying new early detection approaches, developing new ways to make drugs to kill pancreatic cancer and improving the ways in which we get chemotherapies to penetrate the pancreatic cancer cells.

Early Detection The CA19-9 blood test measures a specific sugar that can be elevated in pancreatic cancers and is currently used to gauge pancreatic tumor growth or regression. It is not very useful as a cancer detection screen, because by the time blood levels are high enough to be accurately detected by available methods, the cancer is no longer in its early stages. Dr. Tuveson’s team has developed a strategy to look for sugars similar to CA19-9 that can be used as early detection markers.

Better Therapies One of the most pressing problems in developing therapies for pancreatic cancer is identifying which drugs or combination of drugs will work in clinical trials. To address this, Dr. Tuveson’s team is developing a new way of growing patients cells in the laboratory that they refer to as organoids or “little patients”. This collection of human tissue is designed to mimic the properties of a pancreatic cancer from its earliest stages through aggressive stage IV. In the near future, these “little patients” will hopefully play a key role in helping Dr. Tuveson and his team to develop new therapies for killing pancreatic cancer.

Drug Delivery Before joining the team at the Lustgarten Foundation, Dr. Tuveson discovered that pancreatic cancer tumors develop a protective membrane, or stroma, which prevents chemotherapy from reaching the cancer cells. He concluded that the dismal effectiveness of standard chemotherapy in treating pancreatic cancer could be due to the inability of the drug to reach its target. He and his team, in collaboration with clinicians at Memorial Sloan Kettering Cancer Center, are now starting a clinical trial to test new drugs that poke holes in the stroma and make pancreatic cells accessible to cancer-killing drugs.

UNDERSTANDING THE DISEASE While our understanding of the genetics of pancreatic cancer has increased dramatically over the past decade, there are still many aspects of the disease that we need to study in order to develop optimal treatments for patients.Two grants in 2012 seek a better understanding of how pancreatic cancer grows: Tyler Jacks, Ph.D., The David H. Koch Institute for Integrative Cancer Research at MIT, will seek to understand the specific role of K-ras, a gene that mutates in almost all pancreatic cancers in order to develop better therapies for patients.

George Miller, M.D., New York University School of Medicine, will look for differences in the intestinal bacteria of people with progressive pancreatic cancer as a potential early detection approach for pancreatic cancer.

All living organs function, thrive and grow by a complex interaction with their surroundings. Pancreatic cancers are no different and rely on their surroundings to grow. By understanding these interactions, our scientists can guide us to new ways to stop the disease. The following 2012 grants study these types of questions:

Mikala Egeblad, Ph.D., Cold Spring Harbor Laboratory and Scott Lowe, Ph.D., Memorial Sloan Kettering Cancer Center, will characterize the composition and development of the pancreatic cancer stroma, the membrane surrounding and protecting pancreatic cancers.

Timothy C. Wang, M.D., Columbia University Medical Center, will study the effects of nerve signals and pancreatic cancer in mice and humans.

Another area of study that has largely been ignored up until now is the debilitating muscle wasting that many pancreatic cancer patients face. Muscle wasting, or cachexia, has long been recognized as a direct cause of reduced quality of life that affects up to 85% of pancreatic cancer patients and can make them too weak to continue their cancer therapy. The following two 2012 grants study muscle wasting: Douglas T. Fearon, Ph.D., Cambridge Research Institute, UK, will study how skeletal muscles are affected by pancreatic cancer in order to help patients suffering from muscle wasting.

Teresa A. Zimmers, Ph.D., Thomas Jefferson University Kimmel Cancer Center, will identify which patients are most susceptible to muscle wasting.

Detecting pancreatic cancer early, while it is still operable, continues to be one of the Foundation’s major goals. Several approaches are being pursued by our researchers including developing better imaging technologies that can detect even-smaller pancreatic tumors before they spread as well as blood and other fluid based tests as simple early warning systems for pancreatic cancer. We are pleased to report that for the first time ever, the Lustgarten Foundation is funding two clinical trials to test these early detection techniques in patients.

The Lustgarten Foundation congratulates our esteemed colleague and key scientist, Dr. Bert Vogelstein, M.D., of Johns Hopkins University. He is one of 11 scientists selected for the first Breakthrough Prize in Life Sciences, which recognizes excellence in research aimed at a cure for intractable diseases and extending human life.

Bert Vogelstein, M.D., Johns Hopkins University We are very proud of the fact that the support from the Lustgarten Foundation has helped direct the attention and efforts of Dr. Vogelstein’s team to pancreatic cancer. Prior to 2007, Dr. Vogelstein was peripherally involved in the study of pancreatic cancer, focusing mainly on colon cancer. But with the Lustgarten Foundation’s multi-million dollar commitment to sequencing the Pancreatic Cancer Genome, Dr. Vogelstein, was able to redirect his team at the Johns Hopkins Kimmel Cancer Center to generate the first genetic blueprint of pancreatic cancer which was distinguished as one of the top three 2008 “Breakthroughs of the Year” by Science journal. Based on the results of this initial project Dr. Vogelstein expanded his pancreatic cancer research to developing early detection tests. Over the past three years, Dr. Vogelstein’s team has developed both a blood based DNA test as well as a diagnostic in the laboratory that will be used to predict which pancreatic cysts will become cancerous and which will not, allowing surgeons to remove potentially dangerous cysts before they even develop pancreatic cancer. We are happy to report that Dr. Vogelstein is now bringing together an international group of clinical sites to evaluate these early diagnostic tests in human clinical trials. This project will be conducted in collaboration with 14 medical sites, spanning 5 continents and including over 1,600 patients.

EARLY DETECTION & DIAGNOSIS RESEARCH AND TRIALS

Congratulations to Dr. Ralph Hruban, whose team, including Dr. Vogelstein, won The American Association of Cancer Research Team Science Award. This award deservedly recognizes their amazing work in identifying the genetic components of pancreatic cancer. This achievement was a monumental step forward in the field of pancreatic cancer research and has helped serve as a foundation for further study of the disease.

In addition to looking for blood and cyst based early detection tests, the Foundation is funding a series of grants to develop better imaging techniques for MRI and PET to detect very small pancreatic tumors. The following four 2012 grants develop and test these new imaging techniques:

Hadassa Degani, Ph.D. — Weizmann Institute, will conduct a clinical trial using a new MRI measurement technique that can detect pancreas cancers far earlier than is possible today.



H. Charles Manning, Ph.D., Vanderbilt University, will develop an early pancreatic cancer homing device that can be used for the early detection of pancreatic cancer.



Kenneth Olive, Ph.D., New York-Presbyterian Hospital/Columbia University Medical Center, will test a new imaging approach called Harmonic Motion Imaging (HMI), to look for differences in flexibility and density between pancreatic cancer and normal tissue to improve early detection and diagnosis of the disease.



Roger Y. Tsien, Ph.D., University of California, San Diego, is a Nobel Laureate and world expert on imaging who will develop a new type of contrast agent to be used with standard MRI technology.

While these therapies now give doctors a greater number of options to fight pancreatic cancer, there is still no cure. For this reason, the Foundation continues to fund new drug development research including:

Sangeeta Bhatia, M.D., Ph.D., The David H. Koch Institute for Integrative Cancer Research at MIT and William C. Hahn, M.D., Ph.D., Harvard Medical School Dana Farber Cancer Institute, are working to engineer new therapeutic agents using nanotechnology that can target cancer cells and selectively destroy them.



Sunil Hingorani, M.D., Ph.D., Fred Hutchinson Cancer Research Center, will look for ways to deliver more cancer-killing drugs through the protective barrier that pancreatic cancers form around them.



Jen Jen Yeh, M.D., University of North Carolina School of Medicine, will study why patients respond differently to chemotherapy with the goal of developing individualized patient treatment plans.

Stand Up to Cancer & Lustgarten Foundation Collaboration The Lustgarten Foundation has joined forces with Stand Up To Cancer to fight pancreatic cancer. The aim of this collaboration was to ensure that The SU2C Pancreatic Cancer Dream Team was able to continue their groundbreaking work and move their innovative findings into clinical trials. This dream team was the largest group brought together by SU2C and is led by world-renowned scientists Dr. Craig Thompson, CEO of Memorial Sloan Kettering Cancer Center and Dr. Daniel Von Hoff, Physician in Chief, Translational Genomics Research Institute. During their first 3 years, this group pushed forward 5 clinical trials in pancreatic cancer patients as well as developing and testing a panel of new therapies in the laboratory that may be promising anticancer therapies in patients. Because of this new partnership between SU2C and the Lustgarten Foundation, the Pancreatic Cancer Dream Team will continue its work by initiating 3 additional clinical trials testing newly developed therapies for the treatment of pancreatic cancer.

Projects in 2012 also focus on immunotherapy. Scientists have now found ways to help train the immune system to recognize cancer as an enemy with some very promising success in other cancers. Two 2012 grants use this approach for pancreatic cancer, they are: Carl June, M.D. and Gregory Beatty, Ph.D., Abramson Family Cancer Research Institute at the University of Pennsylvania, have developed a new immunotherapy approach that has been used successfully in leukemia patients. They are now using this same technology to target pancreatic cancer in a human clinical trial. This study is co-funded by the Cancer Research Institute. By leveraging the Lustgarten Foundation’s commitment to pancreatic cancer research with CRI’s mission to bring new immunotherapies to patients, the two organizations aim to achieve greater impact in advancing pancreatic cancer treatment than either could alone. Stephen Thorne, Ph.D., University of Pittsburgh Cancer Institute, will produce and test a new type of immunotherapy (based on the smallpox vaccine) to boost a person’s immune system to fight pancreatic cancer.

All cells have to nourish themselves to live and divide, known as cellular metabolism. Cancer cells need to consume large quantities of sugars to grow as quickly and aggressively as they do. This leads to a potentially new way to kill cancer cells; by cutting off their fuel supply – which is the focus of two of our 2012 research grants:

Dafna Bar-Sagi, Ph.D., New York University School of Medicine, is looking for ways to block the ability of pancreatic cancer cells to take in nutrients from their environment in order to starve them. Alec Kimmelman, M.D., Ph.D., Dana Farber Cancer Institute; Lewis Cantley, Ph.D., Beth Israel Deaconess Medical Institute; Luke Lairson, Ph.D., Scripps Research Institute, are working together to develop drugs that block nutrients from reaching pancreatic cancers in order to starve them.

Patient & Caregiver Information PATIENT AND CAREGIVER EDUCATION (P.A.C.E.) Knowledge Is power. Understanding Fuels Hope. The Lustgarten Foundation remains steadfast in its commitment to provide pancreatic cancer patients and their families with the information they need to make informed decisions. The Lustgarten Foundation launched the P.A.C.E. Program to deliver vital information to this long-underserved group — and with it, reinforcing belief, understanding and hope. This important initiative includes a full-time Program Coordinator who handles all patient calls. The P.A.C.E. Program Coordinator is available to address patient inquiries and concerns, and to provide non-medical information and referrals to a range of supportive community services. The P.A.C.E. Program offers a comprehensive patient information packet that includes a variety of helpful and hopeful resources: Understanding Pancreatic Cancer: A Guide for Patients and Caregivers This handbook helps patients and their caregivers understand the unique challenges associated with a diagnosis of pancreatic cancer. The handbook is a summary of the growing amount of information available and is based upon the premise that being knowledgeable about the challenges that a pancreatic cancer diagnosis poses will better equip patients to make informed decisions about all aspects of treatment and care. An appendix is included that contains a list of resources and a listing of centers designated by the National Cancer Institute as the best cancer facilities, along with a tear-off addressing: What To Ask Your Doctor.

Ask An Expert Patient Education and Information: Series of 19 Publications In 2003, The Lustgarten Foundation launched the Ask an Expert series that features a talk with an expert medical professional (physician/clinician) in their field of expertise. The series offers in-depth information of particular interest to pancreatic cancer patients. Topics to date include:

The Service offers pancreatic cancer patients free and unlimited access to current verified information as they pursue clinical trial opportunities. A unique and key element to the service is providing direct and ongoing telephone support throughout the clinical trial search process.

Renowned Authors Join the Fight Authors Jai Pausch and Will Schwalbe both released new books in 2012 about losing a loved one to pancreatic cancer. Jai’s memoir, Dream New Dreams, is a vivid portrayal of losing her husband, Last Lecture author Randy Pausch, to the disease. Will Schwalbe’s moving book, The End of Your Life Book Club, depicts the relationship between he and his remarkable mother as they face her battle with pancreatic cancer through the power of the written word.
