Mazzini” Hospital, Teramo, Italy Objectives: The therapeutic approach to flail chest continues to generate discussion. The purpose of study was to propose the combination of protective mechanical ventilation and surgical treatment to improve outcomes in flail chest (FC) patients. Methods: From January 2006 to December 2013, we observed 644 chest trauma patients, 27 of whom (4.2%) showed a flail chest. Injury Severity Score was 36 ± 9 and chest Abbreviated Injury Scale was 3 ± 1; all patients displayed a pulmonary contusion. Respiratory failure (PaO2 50 mmHg; SaO2
