 INSTRUCTIONS:
This
form
is
to
be
completed
by
the
primary
faculty
supervisor
of
a
GSFS
final
project
 during
 the
 grading
 period.
 The
 completed
 form
 should
 be
 submitted
 to
 Ms.
 Linda
 Pardee,
 GSFS
 Administrative
 Technician/Department
 Secretary.
 The
 completed
 form
 should
 be
 (1)
 attached
 to
 a
 copy
of
the
student’s
final
paper
or
to
alternative
documentation
if
the
final
project
does
not
take
the
 form
of
a
paper.
The
student’s
name
must
be
redacted
with
a
marker
before
submission
to
the
GSFS
 office.
(2)
The
GSFS
office
also
requires
a
brief
consent
form
signed
and
dated
by
each
student
giving
 us
 permission
 to
 copy
 and
 keep
 capstone
 final
 project
 documentation
 for
 assessment
 and
 program
 review
 (form
 available
 on
 the
 GSFS
 website).
 The
 student
 should
 receive
 a
 copy
 of
 this
 completed
 assessment
rubric
with
their
regularly
graded
final
project.
