Hospital Universitario Vall d´Hebron, Barcelona, Spain Abstract: Introduction: 62 year old woman with frequent and symptomatic junctional ectopy. Methods: N/A Results: A 3D electroanatomic map of the right atria was performed with the EnSite NavX system (Endocardial Solutions, St Paul, Minn) (fig 2). A Halo catheter (Cordis Webster, Diamond Bar, Calif) was positioned along the conduction system (CSys). A His deflection preceded ventricular and atrial depolarization in the ectopic beats (EB) (fig 1). The earliest atrial activation during the EB was observed in the AV node area (fig 2). Dual AV nodal physiology was present and a single nodal echo beat could be induced. Radiofrequency was initially delivered in the slow pathway region (red dots) during which accelerated junctional rhythm was observed without any effect in the EB. Cryoablation (Cryo) in the CSys was then used (orange dots). An application of Cryo in the point of the earliest His (blue dot) made disappear the EB. An incomplete right bundle block (normal HV) was provoked. Conclusions: Application of Cryo in the CSys can be safe and effective in the

treatment of junctional EB.
