‫اﻟﻤﺮاﻗﺒﺔ اﻟﻨﺴ ﺠ ﺔ ﻟﻠﺘﺎﺛ ﺮات اﻟﻮاﻗ ﺔ ﻟﻠﻘﻠﺐ ﻟﻜﻞ ﻣﻦ ﺳﻠﻔﺎت اﻟﻤﻐﻨ ﺴ ﻮم‬ ‫واﻟﺒﺎ ﻮﻛﻠ ﺘﺎزون واﻻﺣﻤﺎض اﻟﺸﺤﻤ ﺔ اوﻣ ﻜﺎ‪ ٣-‬ﻓﻲ اﻻراﻧﺐ‬ ‫اﻟﻤﺤﺪث ﻓ ﺎ اﺣﺘﺸﺎء اﻟﻌﻀﻠﺔ اﻟﻘﻠﺒ ﺔ‬ ‫اﻟﺨﻼﺻﺔ‬ ‫ﻌﺘﺒﺮ اﺣﺘﺸﺎء اﻟﻌﻀﻠﺔ اﻟﻘﻠﺒ ﺔ ﻣﻦ اﻻﺳﺒﺎب اﻻﺳﺎﺳ ﺔ ﻟﻠﻮﻓ ﺎت واﻻﻋﺘﻼﻻت ﻓ ﻲ اﻟﻌ ﺎﻟﻢ‪ ،‬وﻛﻤﺤﺎوﻟ ﺔ ﻟﺘﻄ ﻮ ﺮ ﻋ ﻼج ﺳ ﻠ ﻢ‬ ‫وﻓﻌ ﺎل ﻟ ﺬا اﻟﻤ ﺮض اﻟﻘﻠﺒ ﻲ اﻟﺨﻄ ﺮ ﻓﻘ ﺪ ﺗ ﻢ اﺧﺘﺒ ﺎر اﻻﺛ ﺮ اﻟ ﻮاﻗﻲ ﻟﻠﻘﻠ ﺐ ﻟﻜ ﻞ ﻣ ﻦ اﻻﺣﻤ ﺎض اﻟﺸ ﺤﻤ ﺔ اﻻوﻣ ﻜ ﺎ‪٣-‬‬ ‫واﻟﺒﺎ ﻜﻠ ﺘﺎزون وﻛﺒﺮ ﺘﺎت اﻟﻤﻐﻨ ﺴ ﻮم ﻓﻲ ﻧﻤﻮذج اﺣﺘﺸﺎء اﻟﻌﻀﻠﺔ اﻟﻘﻠﺒ ﺔ اﻟﻤﺤﺪث ﺑﺎﻻراﻧﺐ‪.‬‬ ‫ﻓﻘ ﺪ ﺗ ﻢ ‪ ٣٠‬ارﻧﺒ ﺎ ﻣ ﻦ اﻟ ﺬﻛﻮر اﻟﺒﺎﻟﻐ ﺔ اﻟﻤﺤﻠ ﺔ وﻗ ﺪ ﻗﺴ ﻤﺖ اﻟ ﻰ ﺧﻤﺴ ﺔ ﻣﺠ ﺎﻣ ﻊ ﺗﻀ ﻢ ﻣﺠﻤﻮﻋ ﺔ اﻟﺴ ﻄﺮة اﻟﻘ ﺎﺳ ﺔ‬ ‫وﻣﺠﻤﻮﻋ ﺔ اﺣﺘﺸ ﺎء اﻟﻌﻀ ﻠﺔ اﻟﻘﻠﺒ ﺔ اﻟﻐ ﺮ ﻣﻌﺎﻟﺠ ﺔ وﻣﺠﻤﻮﻋ ﺔ اﻟﺒﺎ ﻮﻛﻠ ﺘ ﺎزون‪)١‬ﻣﻠﻐ ﻢ‪/‬ﻛﻐ ﻢ( وﻣﺠﻤﻮﻋ ﺔ ﻛﺒﺮ ﺘ ﺎت‬ ‫اﻟﻤﻐﻨ ﺴ ﻮم ‪)٢‬ﻣﻠﻐﻢ‪/‬ﻛﻐﻢ( وﻣﺠﻤﻮﻋﺔ اﻻوﻣ ﻜﺎ‪ ٢ )٣-‬ﻣﻠﻐﻢ‪/‬ﻛﻐﻢ(‪.‬‬ ‫ﺣ ﺚ اﺳﺘﻤﺮ اﻟﻌﻼج ﻟﻤﺪة ‪ ٦٠‬ﻮم ﻣﻊ اﻟﻤﺮاﻗﺒﺔ اﻟﺴﺮ ﺮ ﺔ ﺧﻼل ‪ª‬ﺬه اﻟﻔﺘﺮة ﺛﻢ اﻟﺘﻘ ﻢ اﻟﻨﺴ ﺠﻲ ﺑﻌﺪ اﻧﺘ ﺎء ﻣﺪة اﻟﻌﻼج‪.‬‬ ‫ﻟﻘﺪ ﺗﺴﺒﺒﺖ ﻛﻞ اﻟﻤﻮاد اﻟﺘﻲ ﺗ ﻢ اﺧﺘﺒﺎر‪ ª‬ﺎ ﺑﺘﺤﺴ ﻦ ﻣﻌﺘ ﺪ ﻓ ﻲ اﻟﺼ ﻮرة اﻟﻨﺴ ﺠ ﺔ ﻣﻘﺎرﻧ ﺔ ﺑﺎﻟﻤﺠﻤﻮﻋ ﺔ اﻟﻐ ﺮ ﻣﻌﺎﻟﺠ ﺔ ﺣ ﺚ‬ ‫اﺣ ﺪث ﻛ ﻞ ﻣ ﻦ اﻻﺣﻤ ﺎض اﻟﺸ ﺤﻤ ﺔ اﻻوﻣ ﻜ ﺎ‪ ٣-‬واﻟﺒﺎ ﻜﻠ ﺘ ﺎزون وﻛﺒﺮ ﺘ ﺎت اﻟﻤﻐﻨ ﺴ ﻮم اﻧﺤﺮاﻓ ﺎ اﻟ ﻰ اﻟ ﺴ ﺎر ﻓ ﻲ ﺷ ﺪة‬ ‫اﻟﻤﺨﻄﻄﺎت اﻟﺒ ﺎﻧ ﺔ ﻟﻨﺴ ﺞ اﻟﻌﻀﻠﺔ اﻟﻘﻠﺒ ﺔ ﻣﻦ ﺣ ﺚ اﻧﺤﺴ ﺎر ﻗﻄ ﺮ اﻟﻤﻨﻄﻘ ﺔ اﻟﻤﺤﺘﺸ ﺔ واﻧﺨﻔ ﺎض اﻟﺨﻼ ﺎ اﻻﻟﺘ ﺎﺑ ﺔ ﻓ ﺎ‬ ‫ﻣ ﻊ ﺗﻔ ﺎوت ﻣﻠﺤ ﻮظ ﻓ ﻲ ﻣﻘ ﺪار اﻟﺘﺤﺴ ﻦ ﺑ ﻦ ‪ ª‬ﺬه اﻟﻤ ﻮاد ﻓﻘ ﺪ اﺣ ﺪث اﻻوﻣ ﻜ ﺎ‪ ٣-‬ﺗﺼ ﻐ ﺮ ﻣﻌﺘ ﺪ ﻟﻤﻌ ﺪل ﻗﻄ ﺮ اﻟﻤﻨﻄﻘ ﺔ‬ ‫اﻟﻤﺤﺘﺸ ﺔ ﻣ ﻦ ‪ ٢٠٠٠‬ﻣ ﺎ ﻜﺮوﻣﺘﺮ ﻓﺒ ﻞ اﻟﻌ ﻼج اﻟ ﻰ ‪ ١٤٠٠‬ﻣ ﺎ ﻜﺮوﻣﺘﺮ ﺑﻌ ﺪ ‪ ٦٠‬ﻮم ﻣ ﻦ اﻟﻌ ﻼج ﻣﻘﺎرﻧ ﺔ ﺑﺎﻟﻤﺠﻤﻮﻋ ﺔ‬ ‫اﻟﻐ ﺮ ﻣﻌﺎﻟﺠﺔ ‪P< 0.05.‬‬ ‫واﻣﺎ ﺗﻘ ﻢ اﻟﻔﺎﻋﻠ ﺔ اﻟﻤﺨﻔﻀﺔ ﻟﻠﺨﻼ ﺎ اﻻﻟﺘ ﺎﺑ ﺔ ﻓﻘﺪ اظ ﺮت ﻛﺒﺮ ﺘﺎت اﻟﻤﻐﻨ ﺴ ﻮم اﻟﺸﺪة اﻻﻋﻠ ﻰ ﻓ ﻲ ‪ ª‬ﺬا اﻟﻤﻌ ﺎر ﺣ ﺚ‬ ‫ﺗﺴﺒﺒﺖ ﻓﻲ ﺧﻔﺾ ﻣﻌﺪل اﻟﺨﻼ ﺎ اﻻﻟﺘ ﺎﺑ ﺔ ﻣﻦ ‪ ٤٥ -/+ ٢٥٠‬ﻓﻲ اﻟﺤﻘﻞ اﻻﻋﻠﻰ ﻟﻠﻤﺠ ﺮ ﻓﺒ ﻞ اﻟﻌ ﻼج اﻟ ﻰ ‪٢٣ -/+ ٩٠‬‬ ‫ﺑﻌﺪ اﻟﻌﻼج وﻣﻘﺎرﻧﺔ ﺑﺎﻟﻤﺠﻤﻮﻋﺔ اﻟﻐ ﺮ ﻣﻌﺎﻟﺠﺔ‪.‬‬ ‫وﻣ ﻦ ﻣﺠﻤ ﻮع اﻟﻨﺘ ﺎﺋﺞ ﻧﺴ ﺘﻨﺘﺞ ان ‪ª‬ﻨﺎﻟ ﻚ ﺗ ﺎﺛ ﺮ واﻗ ﻲ ﻟﻠﻌﻀ ﻠﺔ اﻟﻘﻠﺒ ﺔ ﻣﻌﺘ ﺪ وواﻋ ﺪ ﻟﻼﺣﻤ ﺎض اﻟﺸ ﺤﻤ ﺔ اﻻوﻣ ﻜ ﺎ‪٣-‬‬ ‫وﻛﺒﺮ ﺘﺎت اﻟﻤﻐﻨ ﺴ ﻮم ﻣﻤﻜﻦ ان ﻘ ﻢ اﻛﺜﺮ ﻓﻲ ﻋﻼج اﺣﺘﺸﺎء اﻟﻌﻀﻠﺔ اﻟﻘﻠﺒ ﺔ ﻓﻲ اﻻﻧﺴﺎن‪.‬‬ ‫‪Abstract‬‬ ‫‪Myocardial infarction (MI) is one of the leading causes of morbidity and mortality all‬‬ ‫‪over the world.‬‬ ‫‪In a trial of developing more effective and safe management of this problematic form‬‬ ‫‪of ischemic heart disease, that has several cardioprotective mechanisms, pioglitazone,‬‬ ‫‪Omega-3, and magnesium sulfate have been assessed in a rabbit model of induced‬‬ ‫‪myocardial infarction (MI).‬‬ ‫‪A thirty local domestic adult male rabbits were divided into 5 groups include healthy‬‬ ‫‪control, induced untreated, pioglitazone treated, magnesium treated, Omega-3 treated‬‬ ‫‪group. These drugs were given in a dose of 1 mg\kg , 2 mg\kg and 2 mg\kg respectively‬‬ ‫‪single per day for 60 days with clinical and histopathological assessment along the‬‬ ‫‪treatment course.‬‬ ‫‪All of the tested agents caused significant cardioprotective and cardioregenerative‬‬ ‫‪effects in comparism to the untreated group by improving the autopsy histopathological‬‬ ‫‪figure causing left graphic shift in regard to reducing the infarct thickness and‬‬ ‫‪inflammatory cells infiltration at the end of treatment course P
