Social History: I live with my mom, dad and 4 year old brother, in Decatur, GA. I attend Decatur Elementary School. My school room has a parrot and a cat.
Birth History: I was born at 32 weeks gestation. I was on a ventilator and had trouble with feeding in the NICU. I was in the NICU for 4 weeks.
Medical History: I was hospitalized with: • Viral pneumonia at 1month of age, hospitalized for 2 weeks, • Rota virus infection at 6 months, • Fell on corner of coffee table at 13 months, 3 stiches, no loss of consciousness, • Surgery on ear tubes at 18 months (8/2010)
