OKLAHOMA EDUCATORS HALL OF FAME (OEHF) NOMINATION FORM - 2014 INSTRUCTIONS Purpose:

To identify and honor persons in Oklahoma education who have exemplified the ideals of leadership, service, and/or research in the field of education.

The nominator shall: 1. Complete page 2 of the Nomination Form. 2.

Write a nomination letter addressing: LEADERSHIP AND PROFESSIONAL MODEL - what kind of rapport has this nominee been able to develop with students, fellow educators, parents and citizens of the community? RESEARCH - how has this person contributed, added, and/or implemented new information for the improvement in his/her field of education? SERVICE - how has the nominee demonstrated citizenship, service and civic responsibility in the education community as well as the community in which he/she resides (list specific examples)? OVERALL IMPACT - how has the nominee had an impact on the state educational community to warrant induction into the Oklahoma Educators Hall of Fame?

3. If deceased, submit a curriculum vita using the OEHF guidelines. 4. Obtain up to three letters of recommendation using the OEHF guidelines. 5. A nominee need not be a member of Phi Delta Kappa International (PDK). If s/he is affiliated with a local PDK chapter, please submit this information, postmarked no later than Monday, March 31, 2014, to the OEHF Board Member representing the chapter of which the nominee is a member. (OEHF Board Member list is on page 6 of this form.)

Please complete a curriculum vita with the following information in the order it is outlined. If nominee is deceased, the vita should be completed by the nominator in conjunction with the nominee's family. Date of birth Total number of years spent in the education profession (minimum of 20 years required) Total number of years spent in education in Oklahoma (minimum of 10 years required) Education (college degrees, certificates, etc.) Professional experience Nominations, awards, fellowships, or other distinctions attained Publications and field research, such as personal case studies Professional leadership positions held Impact on the state educational community Innovative educational programs Extracurricular and community involvement Note: A current member of the Oklahoma Educators Hall of Fame Board of Directors shall not be eligible for nomination to the Oklahoma Educators Hall of Fame until at least one year has passed since his/her resignation or departure from the Board.

Please copy these guidelines for each person writing a letter of recommendation. No more than three letters of recommendation may be attached with the nomination form in addition to the letter written by the nominator. Purpose:
