Getting started with the Optimum Business Account Center. An Optimum ID is your key to our services. To take full advantage of your Optimum services, you and your employees will need an Optimum ID or unique username. Creating Optimum IDs to access the Optimum Business Account Center is easy. The first ID created is the Account Manager ID (Primary ID). It provides administrative access to all the services and features in the Optimum Business Account Center, such as setting up email, managing phone features including Rollover Hunting and Time-of-Day Forwarding, delegating management roles to employees, even paying the bill. The Account Manager ID may have been created by our technician at the time of installation. If so, you can sign in now at optimum.net/business.

If the Account Manager ID was not created for you, you can create one online at optimum.net/newid and simply follow the easy online instructions. Getting started Creating your Optimum ID.

• Go to optimum.net/newid. • Once your ID is created, log in at optimum.net/business. When you log in, you will be on the Dashboard of the Optimum Business Account Center.

Creating users and assigning manager roles. Optimum provides options to help your business run efficiently. The Account Manager can create Optimum IDs that are users, delegate management roles so those users can manage different areas of your business and control the way your business is displayed on the WiFi coverage map if your business is an Optimum hotspot. The Account Manager can assign one or more management roles to one user or many users. Manager can manage all features for all lines on the account. They can create individual User IDs and assign • Phone one or more phone line(s) to those IDs. Manager can manage network settings, like port 80 and 25, dynamic DNS and Static IP, assign an email • Network address to a user ID and register up to 4 devices for Automatic Sign In on the WiFi network.

Creating a new user.

• Click on the “USERS” tab on the top navigation bar to enter the User management section of the account center. • Click on “Add a User”. • Enter the Optimum ID you would like and click on “Check Availability”. • Enter the password and confirm password fields. • Enter the first and last name of the employee. • Click on “Finish”. This will save the Optimum ID and take you to the User Details page.

• On the User Details page, you can assign a phone line to the user to manage. • To assign Phone, Network or Hosting Manager roles to the user, click on the “Administrator Privileges” tab.

Submit a request for your business to become an Optimum hotspot. If your business is not an Optimum hotspot it’s easy to become one. Simply submit a request form in the Optimum Business Account Center.

Choose the way your business is displayed on the WiFi coverage map. If your business is an Optimum hotspot the Account Manger can update the way it appears on the WiFi coverage map at any time in the Business Account Center.

Phone Register devices for Automatic Sign In on the WiFi network. Register up to 4 WiFi-enabled devices to automatically connect to the Optimum Network.

• Click on the “INTERNET” tab on the top navigation bar. • Click on “Manage WiFi”. • Click on “Device Management”. • Follow the instructions to register your device. • Once a device is registered it will appear on the Device Management page. • To edit or remove devices from your device list click on “Edit” or “Remove”.

Your phone service provides unlimited calling to the U.S., Puerto Rico, Canada and the U.S. Virgin Islands. To take full advantage of your phone service, log in to the Optimum Business Account Center. The Optimum Business Account Center is simple and easy to use and puts you in control of more than 20 calling features like Rollover Hunting, Time-of-Day Forwarding, Find Me and more. It’s all included at no extra charge.

Rollover Hunting. Rollover Hunting is a feature that is managed by the Account Manager or Phone Manager. It allows incoming calls to your primary phone line to roll over to the next available line and, if all lines are busy or not answered, the call can roll over to voicemail. If a line in the group is busy, the hunt automatically skips that number and moves to the next number in the sequence. It will hunt through all lines in the group once to find an idle line. You can configure multiple hunt groups and the number of rings before the call is routed to the next line. Each hunt group must have a minimum of two lines and a unique primary number. There are three hunt sequence options: Hunting: routes calls to the first idle line in sequential order, starting with the first line in the group to the • Linear last line in the group. You specify the order (sequence) of the lines within the group. Hunting: varies the starting point for the hunt group and allows for an even distribution of calls within • Circular the group. Calls will be routed to the first idle line starting with the line directly after the line on which the last call was completed. The hunt will then follow the order (sequence) of the lines in the group.

• Uniform Distribution: routes the call to the first available line in the group that has been idle the longest. Note: A line that is dedicated to an alarm, credit card, ATM or fax machine should not be included in a Rollover Hunt Group. To learn more about the Rollover Hunting feature, log in to the Optimum Business Account Center at optimum.net/business and click on the “Explore Optimum” link to watch the tutorial.

Enhanced Voicemail. Enhanced Voicemail lets you listen to messages, create personal or corporate greetings and access your voicemail box from anywhere there’s a phone or internet access. In the Optimum Business Account Center, you can manage many advanced voicemail features, including listening to messages online, setting voicemail alerts and sending your messages to your email account. Log in to the Optimum Business Account Center, click on the “Phone” tab and select “Voicemail” from the drop-down menu. Here you will find the complete instruction guide to activate and manage your voicemail online and via telephone.

Time-of-Day Forwarding.

Business Continuity Forwarding.†

Provides a scheduling capability to forward calls to another number, voicemail or a do-not-disturb announcement during specified time intervals. Multiple schedules can be configured that suit your business needs.

Forward calls to a pre-determined number during a power outage or when the connection to the cable modem is lost so you will never miss a call.

Call Forwarding.

Three-Way Calling.

Automatically redirects your incoming calls to another number, voicemail or do-not-disturb announcement. Call Forwarding is only available for telephone numbers within the continental United States.

Add a third party to your conversation.

Call Forwarding on Busy or No Answer. When the phone line is busy or the line is not answered, the call will be redirected to the number you specify, voicemail or a do-not-disturb announcement.

Block incoming calls from up to 32 selected phone numbers as well as all callers who do not show their Caller ID information. Those callers can dial *82 to allow the Caller ID information to show when calling your number while this feature is active.

Selective Call Forwarding.

Private Outbound Calling.

You can create a list of up to 32 phone numbers and have them forwarded to another phone number, voicemail or do-not-disturb announcement.

Turn this feature on via the account center to hide your name and number on all calls you make. You can also hide your number on a per-call basis by dialing *67 before making a call.

Find Me.*†

Directory Listing and Directory Assistance.

Calls to your phone line can simultaneously ring up to three additional phone numbers. The incoming call will be connected to whichever phone answers first.

Your phone service includes a 411 and White Pages directory listing (simple, straight line listing) for new phone number(s). A 411 and White Pages directory listing for your toll-free numbers is optional and provided at an additional charge.

Caller ID. Displays the name and telephone number of the incoming caller, provided they are not blocking that information. This feature requires a Caller ID compatible telephone or display device.

Call Waiting.†

Call Detail Records. Call Detail Records are provided in the Optimum Business Account Center for all incoming and outgoing calls made to and from your phone line. You can view and download call detail records for the current bill period and up to three prior bill periods.

Alerts you with a special tone when someone else is calling you while you are already on the phone.

Enhanced 911.

Caller ID with Call Waiting.†

Allows emergency operators to automatically know the telephone number and address of the dialing party.

Displays the name and telephone number of the incoming call waiting caller, provided they are not blocking that information. This feature requires a Caller ID compatible telephone or display device.

Busy Redial (*66).

Click-to-Call.

When you get a busy signal, dial *66 to use Busy Redial and you will get a call back when the line you are calling is available. Busy Redial is only available for telephone numbers within the continental United States.

Lets you make phone calls right from your internet browser. Just click on the phone number you want from the Optimum Business Account Center and Click-to-Call takes care of the rest.

Call Return (*69).

Outbound Caller ID Name.

Allows you to specify the name that will appear for your number when you make an outbound call. The person you are calling must have a Caller ID compatible telephone or display device.

VIP Ringing.† Designate up to 32 telephone numbers to ring with a second, distinct ring tone.

Block Unwanted Callers.

Lets you identify and dial the last incoming caller to your line.

Toll-Free Service.

PRI.

Toll-Free Service is exclusively offered to phone subscribers. You can receive toll-free calls from anywhere within the U.S., Puerto Rico, Canada and the U.S. Virgin Islands. You can even keep your existing number and port it over at no additional cost. 411 directory assistance and a local White Pages directory listing are available at an additional charge.

PRI (Primary Rate Interface) service provides an ISDN PRI handoff to connect your traditional PBX to the Optimum network. The service supports up to 23 B channels and 1 D channel and up to 100 Direct Inward Dial (DID) numbers. You can port your existing numbers at no extra charge. Call sessions are available from 4 to 23 in one-session increments.

The Account Manager and Phone Manager can manage your Toll-Free Service in the Optimum Business Account Center. Log in at optimum.net/business.

• Point-to-number management is easy. You can have your toll-free calls directed to any phone line

Use the RJ48 cross-connect cable provided by the Optimum technician at the time of installation to connect your PBX. If you are porting your numbers, you or your phone system vendor should make inbound and outbound test calls using the temporary Pilot (main number), as well as DID numbers to confirm the PRI settings are correctly configured. Testing should be done prior to your port date.

• View and download your call detail records for the current billing cycle plus three previous cycles.

International.

SIP Trunking.

Optimum Business offers great international calling rates and can save you money. To make international calls, you must first sign up for an international calling plan. As the Account Manager or Phone Manager, just log in to the Optimum Business Account Center at optimum.net/business and activate the service. You have the flexibility to enable access to international calling on all, or just selected, lines on your account. It’s easy.

SIP Trunking provides a direct IP connection to the Optimum network for your IP PBX, so your calls are reliable and crystal clear. The service supports from 4 to 24 call sessions in one-session increments and up to 100 Direct Inward Dial (DID) numbers. You can port your existing numbers at no extra charge. Use the ethernet cable provided by the Optimum technician at the time of install to connect to your IP PBX. You must have an IP PBX that was certified to operate with the Optimum network and it must be configured according to the configuration guide provided by Optimum Business. You must also log in to the Optimum Business SIP Trunk Adaptor and configure the LAN side settings. The complete list of certified IP PBXs and configuration guides can be found at optimumbusiness.com/sip.

Call Admission Control allows you to specify the number of sessions on your PRI or SIP Trunk that you want dedicated to inbound and outbound calling. To manage your calling features, log into the Optimum Business Account Center at optimum.net/business.

Phone Important Information. 911 Service.

Business security alarm systems.

Enhanced 911 (e911) is a feature of our phone service that allows emergency operators to automatically know the telephone number and address of the dialing party. This e911 feature has certain requirements in order to operate, as well as certain limitations:

If the Subscriber has a centrally monitored system which is not (i) a high-security monitoring system (UL 681 or similar) or (ii) an emergency medical alert system, the phone will be wired so that it may be available for use with such systems. This request must be made at the time of sale. Although Optimum Business will supply a connection that will allow the operation of the Subscriber’s existing business security system, Optimum Business does not guarantee that any such system will be in complete operational order following the installation of the phone service. As such, it is the Subscriber’s obligation to contact their security system provider to inform them of your Optimum Business installation and any change in phone number, and to request a complete operational test of such system immediately following installation of the phone service. In addition, it is the Subscriber’s responsibility to test the system on a regular basis. Optimum Business installation will ensure proper wiring from the cable modem to the Optimum demark (NID/wiring block), however, any inside wiring from the NID/wiring block to the alarm panel is the Subscriber’s responsibility. In the event of a large-scale power outage or network outage (whether unplanned or maintenance related), the phone service, including any security system which uses the phone service to connect to central station monitoring, will not function.

• The emergency service provider handling the e911 call, and the address the emergency service provider will see, are linked to the address Optimum has on file when the Subscriber originally signed up for service. If the Subscriber does not correctly identify where the Subscriber’s voice-enabled cable modem is actually located, or if the voice-enabled cable modem is moved away from the original address, the Subscriber’s 911 calls may be misdirected to the wrong address or to the wrong emergency service provider.

• IMPORTANT INFORMATION Regarding Keeping Your Phone Number When You Move: Please be aware that during the first 72 hours of arriving at a new service address, if the Subscriber dials 911 from their phone, the Subscriber must relay to the emergency service operator the Subscriber’s new (current) service address. This is necessary to ensure emergency services are dispatched to the Subscriber’s new service address and not the old service address, in the event the emergency services operator may not have the new Registered Location in their records. Therefore, in order to have 911 calls routed correctly, the Subscriber acknowledges responsibility to promptly notify Optimum of any change in service address. Modem power and battery backup. A voice-enabled modem with its own power supply will be provided at no additional charge when installed at the Subscriber’s premises. If there is a local power outage at the Subscriber’s premises, the service will continue to work for as long as the battery is functional. However, if there is a disruption to the cable network or facilities, the service will not work even if the battery is still functional. The Subscriber understands that in such cases it will not be possible to place or receive calls, including calls to access emergency 911 services. It is the Subscriber’s responsibility to notify Optimum if the battery is not working and/or needs replacement. Additionally, some Optimum Business Subscribers may receive notifications regarding the status of their battery backup unit. It is the Subscriber’s sole responsibility to promptly act upon the notification. Optimum will provide the replacement battery at no additional charge.

Optimum does not support the use of the phone service as a connection between (i) medical alert systems or, (ii) any high-security monitoring systems (UL 681 or similar), and the central station monitoring; the Subscriber must maintain an alternate connection. Optimum does not support the use of its phone service as a connection between fire alarm systems (UL 864 or similar) and the central station monitoring for service locations in Pennsylvania. If the phone service is used as a connection between fire alarm systems (UL 864 or similar) and the central station monitoring for a Subscriber service location as permitted hereunder, the Subscriber agrees that it will be solely responsible for contacting its authorized central station monitored fire alarm system provider to test and verify that the central station monitored fire alarm system is in good working order following Optimum’s service installation, including the testing of proper line seizure functionality and the successful transmission of signals at the supervising station.

Medical alert systems. Optimum prohibits the use of its phone service as the connection between medical alert systems and central station monitoring and will neither connect to such services nor provide technical support for the connection.

Static IP (Internet Protocol) service. Optimum Online Boost Plus and Optimum Online Ultra customers have the option of adding Static IP service to their account to meet their business needs. A Static IP address represents the permanent address on the internet assigned to a computer or other device. IP addresses are assigned at the time of installation and cannot be distributed in advance. Using Static IP is the best way for you to host your own web server, mail server, or set up a Virtual Private Network (VPN). Optimum Business offers either one, five, thirteen, or twenty-nine usable Static IP addresses with your Static IP service. The Static IP router is limited to the delivery of our Static IP service. You may connect your own router behind the static router, but cannot replace it. Your Static IP service will not work if you replace or remove the Optimum Static IP router or try to use other equipment. For Network Information, please refer to page 3 of this Guide.

When running a business has you running around town, you can still check email, place orders and research prospective customers from virtually anywhere. With WiFi, you will be able to access the internet at more than 70,000 free Optimum hotspots, including hundreds of commuter rail stations, restaurants and more. Plus, it’s included with your internet service at no additional charge. With WiFi, anytime you or your employees are connected to the internet, you won’t have to worry about going over your cellular data plan limits. Staying connected on the go is easy with WiFi. All Business internet customers and their employees can access WiFi all over the tristate area, as well as other select cities when traveling. Set up is easy. Sign in once and register up to four devices to automatically connect, using your Optimum ID and password. Here’s how:

• Start the WiFi feature on your device. • Select “optimumwifi” as your network and open your browser. • Enter your Optimum ID and Password and choose “Automatic Sign In”. • You can also register and manage your devices in the Business Account Center, see page 12 for more details. Troubleshooting TV Here are some simple troubleshooting instructions so that anyone using your equipment can solve common problems without calling for service.

Problem: No picture.

• Make sure that the TV is turned on and the input is set to the digital cable box. • Verify that all cables are properly and securely connected. • If the digital cable box is plugged into an outlet connected to a wall switch, make sure it is in

• Make sure you have fresh AA batteries in your TV remote. To test your batteries, press any button on your remote and the large circle button will light up.

• Verify that the remote has been programmed to work with your TV. See “Setting Up Your TV Remote” on page 25 for details on programming your remote.

until it shuts down. Release the buttons. The digital cable box will automatically reset. During the reboot process, the digital cable box will display “HOLD” followed by “TURN ON”. This may take three to five minutes.

• Press CBL on the remote to turn the digital cable box on. There may be a slight delay while the menu screen loads.



When the front panel on the digital cable box displays the time, the reboot is complete.

Problem: No sound.

• Verify that the mute button is not pressed on your television remote control or your digital cable box remote control.

• Verify that the volume is turned up. Volume may be adjusted directly from the digital cable box and the TV or TV remote.



Verify that any additional equipment (i.e. DVD or stereo) is properly connected to the digital cable box.

Upon completion of any service or repairs to the electronic equipment (home terminal), the service technician will perform safety checks to determine that the equipment is in proper operating condition. • The modem was qualified under test conditions that included the use of the supplied cables between systems components. Use only the provided power and interface cables and install them properly. • Different types of cord sets may be used for connections to the modem. Use only a main line cord that complies with the applicable modem safety compliance. • Installation of modem must be in accordance with national wiring codes and conform to local regulations. • To reduce the risk of fire, use only CSA certified telecommunication line cord, or national equivalent, for your modem.

Handling Optional, Re-chargeable Battery (for modems). Your modem might include a rechargeable Lithium-Ion battery to provide stand-by operation in the event of an AC power failure. Caution: Do not replace the battery for your modem with an incorrect type, expose the battery to excessive heat, fire or liquid. Any of these conditions can cause product failure and a risk of fire or battery explosion. Do not disassemble, crush or puncture the battery. Keep away from children. Do not incinerate the battery. Recycle or dispose of used batteries promptly and in accordance with local ordinances.

This notice is designed to give you a general understanding of how Optimum protects your privacy in connection with our cable television services (such as Optimum TV), our high-speed internet services (such as Optimum Online), our voice services (such as Optimum Voice) and any other cable service or communication service we may provide to you using our cable system, as such services are defined by applicable law (together “Covered Services”). This notice does not generally cover Optimum’s websites, such as Optimum.com, interactive television channels, such as Optimum Autos and Optimum Homes, or other products or services that may be accessed through Covered Services. These other services are governed by separate privacy policies, which we encourage you to review when using these other services to understand how your information is handled. This notice does not cover Optimum’s Lightpath service. In this notice, “we” and “Optimum” refer to CSC Holdings, LLC. (formerly CSC Holdings, Inc.) and/or any affiliate of CSC Holdings, LLC. that owns, operates, or provides Covered Services. A. How we collect subscriber information. In addition, as part of our normal course of business, we generate and maintain billing and account information, such as your billing history, the services to which you have subscribed, your payment history, your maintenance and repair history, and the kind, location, configuration and MAC addresses of equipment (including TV sets, set-top boxes, CableCARDs, modems, telephones and IP-enabled devices) connected to our facilities. Sometimes, we also obtain additional information from publicly available sources, such as research consultants and marketing firms, to supplement your information. From time to time, we may combine this publicly available information with information that we collect from you. For customer services purposes, we also may keep copies of correspondence concerning your account or records of complaints or inquiries that may be made about our service. In addition, we may sometimes collect responses to customer satisfaction surveys for analytics and research purposes. We may also collect information that helps us detect unauthorized use of our services. We may also collect service-specific Subscriber Information depending on the Covered Services to which you subscribe: • Optimum Voice. If you are a subscriber of a covered Optimum voice service such as Optimum Voice (which is an interconnected Voice-over-Internet Protocol “VoIP” service), we may collect Customer Proprietary Network Information (“CPNI”). CPNI is information that relates to the quantity, technical configuration, type, destination, location and amount of use of an interconnected VoIP service or telecommunications service subscribed to by any customer, and that is made available to the carrier by the customer solely by virtue of the provider-customer relationship; and information contained in the bills pertaining to interconnected VoIP service, telephone exchange service or telephone toll service received by a customer. The disclosure and use of CPNI is subject to specialized restrictions as described in Section F. • Internet Service, including Optimum Online. If you subscribe to a covered Optimum high-speed internet service such as Optimum Online, we may maintain records of Internet Protocol addresses and email accounts that we assign to you, as well as any violations or alleged violations of applicable terms and conditions of usage. We may also collect usage information as it relates to portions of our websites which subscribers may access for the purpose of managing or operating their use of the Covered Services. Additional information about our privacy practices specific to our high-speed internet service can be found in the Additional Privacy Information for Optimum’s High-Speed Internet Service (located at www.optimum.net/Privacy/OOL). • Optimum Digital Cable TV. If you are a digital cable television subscriber, we may also automatically collect information when providing services or when you interact with our systems such as changing your television channel, reviewing listings in an electronic program guide, using your remote or engaging in other interactive programming features. This information is maintained on a de-identified basis, except in connection with billing, troubleshooting, operations or other service related purposes, such as program and content recommendation features and functions, e-Bill or use of certain Optimum Select services, such as requests for information or product purchases.

B. How we use subscriber information. We use Personally Identifiable Information primarily to conduct business activities such as account administration, service provisioning, operations, billings and collections, marketing, customer service, technical support and fraud prevention. These purposes may specifically include the following uses of Personally Identifiable Information: • to help us provide high-quality, competitively-priced Covered Services in an efficient manner; • to ensure that you receive the services to which you have subscribed and execute requests that you make; • to provide you with programming, services and advertising that are of an enhanced value and interest to you; • to facilitate billing and collections functions and obligations; • to provide you with accurate and high-quality customer service, including responding to subscriber inquiries and troubleshooting; • to comply with tax obligations, accounting rules and contractual obligations to vendors; • to ensure that you receive information about products and services that may be of interest to you; • to ensure compliance with applicable laws and the terms, conditions and policies governing the Covered Services; • to protect our rights, such as preventing fraud, service theft and abusive practices; and • to improve our facilities and the services that we provide over them. We may use Subscriber Information to generate de-identified or aggregated information: that is, data from which Personally Identifiable Information has been removed. We may also employ automated processes to generate de-identified or aggregated information from any of our Covered Services. Anonymized or aggregated information does not identify you, and our collection, use, and disclosure of de-identified or aggregated information is not subject to any of the restrictions in this policy and is subject to our discretion. We may use de-identified or aggregated information for programmer, advertiser and internal research and it’s use and disclosure is subject to improve our systems, the content that subscribers receive and their service experience (for example, to measure viewership of channels, TV shows and commercials). We may also use de-identified information to provide targeted content for our services such as programming and advertising, or provide our recommended features or products to you. We may also use this information to deliver relevant third-party advertising/promotional messages using your set-top box or through other IP-enabled device applications without disclosing Personally Identifiable Information to these third party advertisers. Additionally, Optimum provides Subscribers with the opportunity to restrict the use of program and content recommendations in association with certain Optimum services, content recommendations and other of our recommendations, features and functions, by visiting www.optimum.net/ Privacy/Preferences. Although not required to do so, we do offer you a choice as to whether you wish to have targeted third-party advertising/promotional messages delivered to your set-top box. PLEASE CALL US AT 1-888-425-2591 or visit www.optimum.net/Privacy/Preferences. Please note, your preferences may not take effect immediately. Our use of certain Subscriber Information associated with voice service, however, is limited as described in Section F. Individuals who may be authorized to have access to Personally Identifiable Information include our employees, agents, and affiliates within our family of affiliated businesses, including but not limited to Newsday, Madison Square Garden® and Rainbow Media Holdings, though we seek to limit access to individuals on a need to know basis. Sometimes, we may provide access to certain information to select third parties to assist us in providing services or for other legitimate business purposes. For example, we may provide access to certain information to companies providing services to us, including installation and repair, billing, call center, sales and marketing, market research, fulfillment, mailings, data storage, analysis and processing, bill collection, auditing and accounting, credit reporting and legal services. We may also provide access to third parties who assist us in providing targeted advertising and audience analysis initiatives. We will provide access to CPNI for use in sales and marketing only to our agents and affiliates. The frequency of such access depends on our business needs. C. When we disclose subscriber Information. Except as set forth below, Optimum will not disclose Personally Identifiable Information to third parties unless (a) it is necessary for us to render our Covered Services or conduct a legitimate business activity related to our Covered Services; or (b) you provide appropriate consent in advance. If you subscribe to a Covered Service other than voice service, we may furnish mailing list information to marketing organizations, programmers and other businesses. Mailing list information includes your business name, addresses and the non-voice services to which you subscribe (for example, basic tier, HBO). The law allows us to disclose this information unless you tell us otherwise. IF YOU DO NOT WANT US TO DISCLOSE MAILING LIST INFORMATION ABOUT YOU, PLEASE CALL US AT 1-888-425-2089. Please note, your opt-out preferences may not take effect immediately and cannot be applied retroactively. If you subscribe to a covered Optimum voice service such as Optimum Voice, applicable regulations allow us to disclose “Subscriber list information,” which includes your business name, address and telephone number that has been published or accepted for publication in any directory format. For example, we may give your business name, address and number to organizations who publish phone directories or who provide 411 services or similar public services. We may transmit your name and number so that the person you call can see such information on his or her Caller ID. Certain procedures allow you to block transmission of Caller ID information. You may contact us to obtain additional information about these procedures. We may also transmit your business name, number and registered location to a public safety answering point (“PSAP”) when you dial 911 for emergency services. Disclosure of CPNI data is subject to additional specific limitations, as described in Section F. We may also provide your Personally Identifiable Information to third parties when you expressly consent to such disclosure, such as when you make purchases from third-party vendors using our Optimum Select technology or request information from third parties through certain interactive channels.

You acknowledge, consent and agree that we may access, preserve and disclose your Subscriber Information, including your Personally Identifiable Information, your account information and content if required to do so by law or in a good faith belief that such access, preservation or disclosure is reasonably necessary to: (a) comply with legal process; (b) enforce our Terms of Service, Privacy Policy, or other contracts with you; (c) respond to claims that any content violates the rights of third parties; (d) respond to your requests for customer service; (e) respond to inquiries from government agencies regarding customer-initiated complaints provided to such agencies; or (f) protect the rights, property or personal safety of Optimum, its agents and affiliates, its users and the public. We may also disclose information to law enforcement agencies in emergency circumstances, where the disclosure of such information is consistent with the types of emergency disclosures permitted or required by law. Finally, we also reserve the right to disclose and transfer all of your Subscriber Information, including your Personally Identifiable Information, to a successor company in connection with a merger, acquisition or sale of all, or components of, our business. D. We may keep information about you for some time. We generally keep some or all Subscriber Information for as long as necessary for the purpose(s) for which it was collected or as required by law. When information is no longer needed or required to be maintained, we destroy it in accordance with our internal policies. E. You may obtain access to information about you. You also have the right to inspect Personally Identifiable Information that Optimum has collected and maintains in connection with the provision of Covered Services to you. If you would like access to such information, please notify us in writing at the local business office specified on your service bill. We will then contact you to set up an appointment during regular business hours. Please note, Optimum reserves the right to charge you its reasonable costs in connection with retrieving Subscriber Information and providing you with access thereto. In order to obtain CPNI, please see Section F. Subscribers may choose not to receive promotional or marketing telephone calls or postal mailings regarding the Covered Services by contacting us at 1-888-425-2088 for placement on Optimum’s “Do Not Call” list or by contacting us at 1-888-425-2090 for placement on Optimum’s “Do Not Mail” list. Please note, any Subscriber requests after May 1, 2010 to be placed on the “Do Not Mail” list will only be effective for the 12 month period following such request. If Subscribers wish to continue to remain on the “Do Not Mail” list, Subscribers will need to make another request upon the expiration of the previous request. Please also note, “Do Not Call” and “Do Not Mail” requests may take up to thirty (30) days to become effective. F. Special rules about telephone-related information. Under federal law, you have a right, and we have a duty, to protect the confidentiality of a category of telephone-related information known as Customer Proprietary Network Information or “CPNI.” Optimum restricts access to your CPNI. For example, call-detail records, which contain information about the calls your business makes, are available online. In order to access call-detail records online, you must have an Optimum ID and password. All other CPNI is restricted by requiring a password and by requiring that your business provides certain non-public verification information over the phone to confirm your identity. A written request may also be used to obtain your CPNI. You must send the written request to Cablevision OV, 6 Corporate Center Drive, Melville, New York 11747, Attn. Shred Services – CDR. All requests will be verified. Optimum also restricts the use of your CPNI. Unless we have your express permission, Optimum does not share your CPNI with non-affiliates for marketing purposes. We will, however, use such information to tailor your service and to market related Optimum Voice products and services to you. G. Our collection and use of information is subject to legal limitations. The Cable Act imposes limitations with respect to the collection and disclosure of Personally Identifiable Information by cable operators, unless you provide us consent to collect or disclose information in another fashion. Generally speaking, cable operators may not use the cable system to collect personally identifiable information concerning Subscribers without prior consent of the Subscriber concerned except to obtain information necessary to render our cable service or other services to our Subscribers or to detect unauthorized reception of cable communications. In addition, subject to certain specified exceptions, cable operators generally may not disclose Personally Identifiable Information without consent of the Subscriber concerned. Also, cable operators must take such actions as are necessary to prevent unauthorized access to such information by a person other than the Subscriber or cable operator. If we violate your rights, you may be entitled to bring a civil action in a federal court, which may award actual, liquidated, and punitive damages, fees and costs, and other remedies that may be available. In addition, if anyone asks us to provide information about your business, we may have to notify you, and you may have the right to appear in court. H. How we protect subscriber information. We employ physical, electronic and procedural safeguards to protect Subscriber Information. For example, we utilize secure socket layer (SSL) encryption to protect certain information you provide to us; employ verification measures to protect email during delivery; maintain certain subscriber databases in restricted areas and secure the content by use of firewalls and other security methods. We also limit access to databases containing Subscribers’ Personally Identifiable Information to specifically authorized employees and agents and other parties identified in the disclosure section. Please see Section C. Despite these measures, we do not guarantee the security of information stored in our database or that unauthorized parties will not intercept information you send or receive over the internet. As a shared resource, the internet subjects our services and you to the risk of unauthorized access to files and data, even when security measures are implemented. If you use Optimum’s high-speed internet service such as Optimum Online, to send, receive or post personal or confidential information, you do so at your own risk. I. Questions about the policy. We may furnish certain de-identified information, on either an aggregate or non-aggregate basis, regarding viewing habits and system interaction to third parties for purposes that may include, but not be limited to, providing you with more relevant programming, advertising and features, increasing the value of the Covered Services, or providing you with information or offers about products or services that we believe may be of interest to you.

J. Updates to the policy. We will review this Privacy Policy periodically as we deem appropriate and when we change or update Covered Services. We will notify you of any material changes by posting the revised policy on our Optimum website or by written or electronic correspondence. Your use of the Covered Service(s) following the notice of a revised policy constitutes your acceptance of the revised Privacy Policy. General terms and conditions of service In addition to these General Terms and Conditions of Service, Subscriber agrees to be bound by the terms of service for the applicable Optimum service as set forth at www.optimum.net, such as Optimum TV, Optimum Online and Optimum Voice, as well as the Optimum Customer Privacy Notice, as such may be updated from time to time (collectively, the “Terms of Service”), which are incorporated herein by this reference. In the event of any conflict between these Terms and Conditions below and the Terms of Service, the Terms of Service shall control. Note: This agreement contains a binding arbitration provision, which includes a waiver of class actions and provisions for opting out of arbitration, in Section 18 that affects your rights under this agreement with respect to all services. 1. Payment of charges: Subscriber will be billed monthly in advance for Services to be received, plus pro-rata charges, if any, for periods not previously billed. Subscriber will be billed monthly for other Services ordered where charges are based on actual usage or on orders placed during the previous month. Subscriber agrees to pay all undisputed monthly charges and all applicable fees and taxes as itemized on the Optimum monthly bill and notify Optimum of disputed items within thirty (30) days of receipt, or longer as provided by applicable law. Failure to pay charges billed, including checks returned for insufficient funds, may result in discontinuance of Service, the removal of all Optimum Equipment as defined below, and/or imposition of a late payment or service charge. If the Subscriber has more than one account, Business and/or Residential, served by Optimum, all Optimum provided Services at all locations may be subject to discontinuance of Service in the event any one account remains unpaid. In the event collection activities are required, an additional collection charge may be imposed. 2. Optimum property: All equipment, including but not limited to, any cables, wires, amplifiers, cable boxes, access cards, remotes, cable cards, battery backup units, modems, and routers distributed to and/or installed for use in the Subscriber’s service location(s) by or on behalf of Optimum (“Equipment”) remains the property of Optimum. None of the Equipment shall become a fixture. Optimum property is intended to service and reside at the specific service location and is not to be used off premises without Optimum authorization. Subscriber must return all Equipment upon substitution of use or termination of Service. Failure to do so will result in a charge to be determined in accordance with Optimum’s then current schedule of charges for non-returned Equipment, which amount shall be due immediately. Subscriber agrees to pay such charge whether the Equipment is lost (through theft or otherwise), damaged or destroyed. 3. Disruption of service: All Optimum Services are provided on an “AS IS” and “AS AVAILABLE” basis. In no event shall Optimum be liable for any failure or interruption of Service, including, without limitation, those failures and interruptions resulting in part or entirely from circumstances beyond Optimum’s reasonable control. Subject to applicable law, credit, with respect to Subscriber’s recurring monthly subscription fee, shall be given for qualifying outages of Optimum Services. 4. Repair of Optimum equipment: Optimum will repair and/or replace defective Equipment, if any, as long as such damage was not caused by misuse or other improper operations or handling by Subscriber. Optimum is not responsible for the maintenance or repair of Subscriber-provided equipment, including but not limited to television sets, or other video equipment, computers, modems, or any other related Subscriber-provided equipment. Optimum makes no warranties, with respect to Equipment or Service provided by Optimum or with respect to the Equipment’s compatibility with any Subscriber-provided equipment. 5. Subscriber property: Optimum assumes no responsibility, and shall have no responsibility, for the condition or repair of any Subscriber-provided equipment and/or software. Subscriber is responsible for the repair and maintenance of Subscriber-provided equipment and/or software. Optimum is not responsible or liable for any loss or impairment of Optimum’s Service due in whole or in part to a malfunction, defect or otherwise caused by Subscriber-provided equipment and/or software. 6. Taxes: Subscriber agrees to pay any local, state or federal taxes imposed or levied on or with respect to the Services, the Equipment or installation or service charges incurred with respect to the same. 7. Care of Optimum property: Subscriber agrees that neither Subscriber nor any other person, except Optimum’s authorized personnel, will open, tamper with, service, make any alterations to, or remove any Equipment from the service address of initial installation. Any alteration, tampering, removal, etc., or the use of equipment which permits the receipt of Services without authorization, or the receipt of Services to an unauthorized number of outlets, or to unauthorized locations constitutes theft of service and is prohibited. 8. Access to subscriber premises: Subscriber authorizes Optimum and its employees, agents, contractors and representatives to access and otherwise enter the Subscriber’s premises to install, inspect, maintain and/or repair the Equipment and, upon the termination of Service, to remove the same from the premises. Optimum’s failure to remove its Equipment shall not be deemed an abandonment thereof. 9. Assignment or transfer: This Agreement and the Services and/or Equipment supplied by Optimum are not assignable or otherwise transferable by Subscriber, without specific written authorization from Optimum. 10. Termination of service: Unless otherwise terminated in accordance with the terms hereof or the Terms of Service, this agreement shall automatically renew on a month-to-month basis. Subscriber acknowledges that upon such renewal all pricing is subject to change. 11. Breach of agreement: In the event of any breach of this agreement (including the Terms of Service) by Subscriber, the failure of Subscriber to abide by the rates, rules and regulations of Optimum, the failure of Subscriber to provide and maintain accurate registration information, or any illegal activity by the Subscriber using the Service, this agreement may be, at Optimum’s option, terminated and Optimum’s Equipment removed. Subscriber shall pay reasonable collection and/or attorney’s fees to Optimum in the event that Optimum shall find it necessary to enforce collection or to preserve and protect its rights under this agreement. 12. Security deposit: Any security deposit given by Subscriber for the Equipment or Optimum’s Service will be due and payable upon the first monthly billing. Such security deposits will be returned to Subscriber within sixty (60) days of termination of Optimum’s Service so long as payment has been made for all amounts due on Subscriber’s account and Subscriber has returned the Optimum Equipment undamaged. 13. Content and services: All content, program services, program packages, number of channels, channel allocations, broadcast channels, interactive services, email, data offerings and other services are subject to change in accordance with applicable law. 14. Rates: All rates are subject to change in accordance with applicable law.

15. Late Fee: If Subscriber account is 30 days past due, a reminder message will be included on Subscriber monthly bill. If the past due balance remains unpaid, Subscriber may be charged an applicable late fee in addition to Subscriber past due balance at Optimum’s then current rate. If Subscriber account remains unpaid Subscriber Services may be disconnected. Subscriber can avoid incurring late fees by paying Subscriber monthly bill promptly. Any late fees assessed are not considered interest or penalties. Optimum expects that Subscriber will pay for Services on a timely basis, and Optimum does not extend credit to customers. 16: Disclaimer: Optimum assumes no liability for any program, services, content or information distributed on or through the Services and Optimum expressly disclaims any responsibility or liability for Subscriber use thereof. Further, Optimum shall not be responsible for any products, merchandise or prizes promoted or purchased through the use of the Services. 17. Right to make credit inquiries: Subscriber authorizes Optimum to make inquiries and to receive information about Subscriber credit experiences, including Subscriber credit report, from others, to enter this information in Subscriber file, and to disclose this information concerning you to appropriate third parties for reasonable business purposes. 18. Arbitration: Except as noted below under “Excepted Claims,” any and all disputes arising between Subscriber and Optimum, or related to or arising from Subscriber relationship with Optimum, including the validity, enforceability, or scope of this Arbitration Provision (with the exception of the enforceability of the class action waiver clause), shall be subject to binding arbitration in accordance with this Arbitration Provision. Resolving Subscriber dispute with Optimum through arbitration means Subscriber will have a fair hearing before a neutral arbitrator instead of in a court by a judge or jury. Subscriber AGREES THAT BY ENTERING INTO THIS AGREEMENT, Subscriber AND OPTIMUM EACH WAIVE THE RIGHT TO A TRIAL BY JURY AND TO PARTICIPATE IN A CLASS ACTION. IF Subscriber DOES NOT WISH TO BE BOUND BY THIS ARBITRATION PROVISION, Subscriber MUST NOTIFY OPTIMUM IN WRITING WITHIN 30 DAYS OF THE DATE THAT YOU FIRST RECEIVE THIS AGREEMENT BY EMAILNG US AT [email protected] OR BY MAIL TO CABLEVISION RESEARCH & SUPPORT, 200 JERICHO QUADRANGLE, JERICHO, NY 11753 ATTN: ARBITRATION. Subscriber WRITTEN NOTIFICATION TO OPTIMUM MUST INCLUDE Subscriber BUSINESS NAME, ADDRESS, AND OPTIMUM ACCOUNT NUMBER AS WELL AS A CLEAR STATEMENT THAT Subscriber DOES NOT WISH TO RESOLVE DISPUTES WITH OPTIMUM THROUGH ARBITRATION. Subscriber DECISION TO OPT OUT OF THIS ARBITRATION PROVISION WILL HAVE NO ADVERSE EFFECT ON Subscriber RELATIONSHIP WITH OPTIMUM OR THE DELIVERY OF OPTIMUM SERVICES TO Subscriber. IF Subscriber HAS PREVIOUSLY NOTIFIED OPTIMUM OF Subscriber’S DECISION TO OPT OUT OF ARBITRATION, Subscriber DOES NOT NEED TO DO SO AGAIN. Because the Service(s) provided to Subscriber concerns interstate commerce, the Federal Arbitration Act (“FAA”), not state arbitration law, shall govern the arbitrability of all disputes under this Arbitration Provision. Any state statutes pertaining to arbitration shall not be applicable. Subscriber remedies, and damages arising between Subscriber and Optimum. The arbitration will be administered by the American Arbitration Association (“AAA”), 335 Madison Ave., Floor 10, New York, NY 10017-4605, 1-800-778-7879, www.adr.org under the AAA’s Commercial Dispute Resolution Procedures and the Supplementary Procedures for Consumer Related Disputes. If the AAA will not enforce this Arbitration Provision as written, it cannot serve as the arbitration organization to resolve Subscriber dispute. If this situation arises, or if the AAA for any reason cannot serve as the arbitration organization, the parties shall agree on a substitute arbitration organization or ad hoc arbitration, which will enforce this Arbitration Provision as to the dispute. If the parties are unable to agree, the parties shall mutually petition a court of appropriate jurisdiction to appoint an arbitration organization or ad hoc arbitrator that will enforce this Arbitration Provision as written. If there is a conflict between this Arbitration Provision and the AAA rules, this Arbitration Provision shall govern. A single arbitrator will resolve the dispute between Subscriber and Optimum. Participation in arbitration may result in limited discovery. The arbitrator will honor claims of privilege recognized by law and will take reasonable steps to protect confidential or proprietary information, including Subscriber personally identifiable information. All issues are for the arbitrator to decide, including the scope of this Arbitration Provision, but the Arbitrator is limited and bound by terms of this Arbitration Provision. The arbitrator will make any award in writing but need not provide a statement of reasons unless requested by a party. An award rendered by the arbitrator may be entered in any court having jurisdiction over the parties for purposes of enforcement. Unless Subscriber agrees otherwise, any arbitration hearings will take place in a location convenient to Subscriber in the area where Subscriber receives the Service(s). If the amount in dispute is less than $50,000, Optimum agrees that Subscriber may choose whether the arbitration is conducted solely on the basis of documents submitted to the arbitrator on a telephonic hearing, or by an in-person hearing as established by AAA rules. If an award granted by the arbitrator exceeds $75,000 or includes any form of injunctive relief, either party can appeal that award to a three-arbitrator panel administered by AAA by a written notice of appeal within thirty (30) days from the date of entry of the written arbitration award. An award of injunctive relief shall be stayed during any such appeal. The members of three arbitrator panel will be selected according to AAA rules. The threearbitrator panel will issue its decision within one hundred and twenty (120) days of the date of the appealing party’s notice of appeal. The decision of the three-arbitrator panel shall be final and binding, except for any appellate right that exists under the FAA. Subscriber agrees that if Subscriber fails to contact Optimum within one (1) year of the date of the occurrence of the event or facts giving rise to a dispute, Subscriber waives the right to pursue, in any forum, including arbitration or court, a claim based upon such event, facts or dispute. Subscriber AGREES TO ARBITRATE Subscriber DISPUTE ON A SOLELY INDIVIDUAL BASIS; CLASS ARBITRATIONS AND CLASS ACTIONS ARE NOT PERMITTED. Subscriber and Optimum agree that the other may bring claims against the other only in Subscriber’s or its individual capacity and not as a plaintiff or class member in any purported class or representative proceeding. This Arbitration Provision does not permit, and explicitly prohibits, the arbitration of consolidated or class action disputes. No claim may be brought on behalf of the general public or as a private attorney general or on behalf of other Subscribers or similarly situated persons unless the statute Subscriber is suing under provides for such actions. No claim subject to arbitration hereunder may be combined with a claim subject to resolution before a court of law. The arbitrator may award declaratory or injunctive relief only in favor of the individual party seeking relief. Such injunctive relief may only be awarded to the extent necessary to (i) provide relief warranted by that party’s individual claim and (ii) to return that party to the position it occupied before its claim arose. The arbitrator’s authority to award injunctive relief is limited solely to the relationship between the claimant and Optimum. The arbitrator

Optimum Business Benefits may not award injunctive relief to any person or entity other than the claimant or that would interfere with the operation of the cable system. Further, the arbitrator may not consolidate more than one person’s claims and may not otherwise preside over any form of a representative or class proceeding. OPTIMUM WILL ADVANCE ALL ARBITRATION FILING FEES AND ARBITRATOR’S COSTS AND EXPENSES UPON Subscriber’s WRITTEN REQUEST PRIOR TO THE COMMENCEMENT OF THE ARBITRATION. Subscriber IS RESPONSIBLE FOR ALL ADDITIONAL COSTS THAT Subscriber INCURS IN THE ARBITRATION, INCLUDING, BUT NOT LIMITED TO, ATTORNEYS OR EXPERT WITNESSES. IF THE ARBITRATION PROCEEDING IS DECIDED IN OPTIMUM’S FAVOR, Subscriber SHALL REIMBURSE OPTIMUM FOR THE FEES AND COSTS ADVANCED TO Subscriber ONLY UP TO THE EXTENT AWARDABLE IN A JUDICIAL PROCEEDING. IF THE ARBITRATION PROCEEDING IS DETERMINED IN Subscriber’s FAVOR, Subscriber WILL NOT BE REQUIRED TO REIMBURSE OPTIMUM FOR ANY OF THE FEES AND COSTS ADVANCED BY OPTIMUM.

IF A PARTY ELECTS TO APPEAL AN AWARD TO A THREE-ARBITRATOR PANEL, THE PREVAILING PARTY IN THE APPEAL SHALL BE ENTITLED TO RECOVER ALL REASONABLE ATTORNEYS’ FEES AND COSTS INCURRED IN THAT APPEAL. NOTWITHSTANDING ANYTHING TO THE CONTRARY IN THIS ARBITRATION PROVISION, OPTIMUM WILL PAY ALL FEES AND COSTS THAT IT IS REQUIRED BY LAW TO PAY. If the ban on class action or class arbitration contained in this Arbitration Provision is held to be unenforceable, then the sections of this Arbitration Provision requiring arbitration of claims will be unenforceable, and the claim will be decided in court. In that instance, or any instance when a claim between Subscriber and Optimum proceeds to court rather than through arbitration, Subscriber and Optimum each waive the right to any trial by jury through this Agreement.

If any portion of this Arbitration Provision besides the ban on consolidation and class actions is determined to be unenforceable, then the remainder of this Arbitration Provision shall be given full force and effect. The terms of this paragraph of the Arbitration Provision shall survive termination, amendment or expiration of this Agreement. For purposes of this Agreement, the term “Excepted Claims” shall mean claims (a) relating to a party’s intellectual property rights; (b) relating to the unauthorized use, theft, or piracy of products or services; (c) in which the aggregate amount in controversy may be heard in a small claims court in Subscriber’s jurisdiction, in which case either party can proceed to that court; or (d) which a party may bring before a regulatory authority or agency empowered by applicable law, provided, however, that the claim is not a collective or class action or brought on behalf of the general public, as a private attorney general, or in any other representative capacity.

19. Entire agreement: These Terms and Conditions (including the Terms of Service) constitutes the entire agreement between the Subscriber and Optimum. No undertaking, representation or warranty made by an agent or representative of Optimum in connection with the sale, installation, maintenance or removal of Optimum’s Services or Equipment shall be binding on Optimum except as expressly included herein. CENTRAL STATION MONITORED ALARM SERVICE LIMITATIONS. Although we will provide a wire connection so that Optimum Voice services are available for use with Subscriber’s existing security and fire systems, it is Subscriber’s responsibility to follow-up with Subscriber’s alarm company to confirm that they have Subscriber’s correct and current phone number and to perform any periodic operational tests that Subscriber’s central station-monitoring provider might recommend. Subscriber further understands that in the event of an outage, Subscriber’s Optimum Voice service will not be available to contact Subscriber’s central station-monitoring provider. Optimum does not support the use of any Optimum Voice service as a connection for (i) emergency medical alert systems or (ii) all high-security monitoring systems (UL 681 or similar). If Subscriber has any such systems, Subscriber must use an alternative connection.
