Our Lady of the Assumption Church Wedding Music Selection Sheet Congratulations on your upcoming wedding! Please complete this form referencing the music CD you will/have received during your wedding preparation meeting. The CD contains most commonly requested hymns for weddings and which are intended to correspond well with your selected readings. Any desired selections not on the list should be approved by the music director. It is strongly recommended couples requesting vocalists and/or instrumentalists do so as soon as possible to help ensure proficient musicians and vocalists can be scheduled. Guest musicians and vocalists should be approved by the music director beforehand.
