The EPBB PSEGLI Solar PV rebate will be calculated in accordance with the PSEGLI Incentive Calculator in rebate amounts that are offered at the time received. Commercial entities are eligible to be pre-approved for up to $66,000 (or the maximum total rebate offered at the time the application is received per 12 month period). Multiple applications will be accepted, but the combined solar PV Systems are not to exceed $66,000 (or the maximum total rebate offered at the time the application is received per 12 month period) per entity.

Terms and Conditions

To be eligible for the PSEGLI commercial Solar PV Program rebate, you must be an existing PSEGLI Customer with regard to the site of the installation. 2 The Customer must also own the property where the PV System will be installed.

3 A Non-refundable Application Fee of $100 is required in order to process this application and receive pre-approval for the project. The Application Fee must be submitted in the form of a Check or money order, Payable to PSEGLI, and mailed to PSEGLI Commercial Solar Entrepreneur Program, Rebate Processing, 15 Park Avenue, Melville, NY 11747. 4 The Customer and Installer must sign the Rebate Application to be eligible for PSEGLI Commercial Solar PV Program rebate. 5 The Rebated Solar PV System is required to remain at the address listed on this application. The Customer must maintain the PV System in good condition compliant with PSEGLI’s Red Book Specifications and Requirements for Electric 6 Installation 7 The Customer is responsible for obtaining all required permits (i.e. building, electrical, etc.). 8 The Customer and Contractor are responsible for any updates needed to the application. 9 Only new, grid connected, PV Systems and current Underwriters Laboratories 1741 Certified PV inverter equipment, are eligible for a Rebate. 10 To ensure program compliance, the contractor must show the Total System Price of the PV system in Installation Sales Contract. Total System Price shall include any and all costs associated with the complete installation of the proposed PV system (I.e. all equipment, labor, and margin) in the Installation Sales Contract. No rebates or tax benefits shall be included in this total. Total System Price shall not include non-incidental site preparation, including but not limited to roof repair, structural work to the residence/site, or removal of trees and other obstructions. 11 The Maximum Rebate paid by PSEGLI per metered account will not exceed the rebate calculated for the PV System that is expected to produce no more than 110% of the Customer's annual energy (kWh) for the prior 12 months 12 PSEGLI reserves the right to offer a Rebate at the current Rebate level at the time of the submission of the application and to credit PSEGLI account first. Rebate paid by PSEGLI per metered account will not exceed $66,000 or 50% of the Total System Price of the PV system minus any The Maximum 13 arrears subsidies, whichever is less. 14 PSEGLI (on its own behalf and on behalf of any other entity funding the Rebate) reserves the right to conduct post-installation, follow-up visit(s) or have such visits conducted on its behalf, in order to provide PSEGLI with the opportunity to evaluate the installed PV System and determine the actual kW output, as well as to confirm Program compliance and Rebate eligibility. The results of a PSEGLI approved post-installation inspection may impact the final rebate amount to the Customer. 15 PSEGLI will not be responsible and shall be held harmless and indemnified by the Customer and/or Installer for all liabilities and costs including but not limited to failure to pay or remit sales and use taxes. 16 PSEGLI reserves the right to modify or withdraw this Program and the Rebates payable under the Program at any time and without notice. 17 A Shading Analysis for solar site assessment must be performed and applicants must submit the "Monthly Solar Access" results for all panel arrays. 18 The Customer shall retain all appropriate owner manuals, applications, "one-line" drawings, warranty documents etc. regarding the PV System. 19 Billing credits and/or Rebates will not become effective until the PV System passes PSEGLI’s field inspection & the Net Meter is installed. 20 PV System equipment must carry a minimum of a 5 year warranty on inverters and a 20 year warranty on Panels with a minimum of 80% lifetime output of manufactures designed ratings. 21 PSEGLI reserves the right to base Rebate eligibility and amount on factors such as safety, shading, roof orientation and tilt. Applicants must submit a Google™ Map Satellite view set to view at the lowest elevation. Indicate the four compass points (N, S, E, W) on the Google™ Map Satellite view. 22 The PSEGLI Solar PV Rebate is based on the PV systems expected output. The expected output factors include systems size, type of equipment installed, panel orientation, and shading. The Rebate is calculated using the Expected Performance Based Buy-down method which allows PSEGLI to incentivize more efficient and optimally sited solar systems. The PSEGLI Solar Electric Incentive Calculator is provided on the PSEGLI Web site and is the only tool which will be accepted to calculate the rebate. 23 The Customer is required to meet the eligibility requirements for Net Metering in order to receive a PSEGLI Rebate. (See tariff leaf 34G.). 24 New construction projects will be given a 12 month preapproval timeframe. New construction applicants shall be required to submit a copy of the building permit along with a load letter in kWh for the proposed building. 25 The Customer must submit the Serial Numbers for all Panels & Inverters with pre and post-installation photographs of the PV System and the respective Invoice(s). 26 By initialing below, I hereby consent to the disclosure by PSEGLI or its agents to the ‘Installer/Contractor’ listed on page 1 of this application of electric consumption data in kilowatt hour information for the Account associated with this application. The Electric consumption data in kilowatt hour information may be disclosed to Installer/Contractor via telephone, email, or other electronic means, including through the provision to Contractor of web access. 27 For EPBB applications where either an existing pre-approval exists under the old $/(DC)watt rebate or in instances where the customer already has a previously rebated system, the EPBB PSEGLI Solar PV rebate will be calculated in accordance with the PSEGLI Incentive Calculator in rebate amounts that are offered at the time received. For any single metered account, the maximum eligible total system rebate will be based on the cumulative amount of previously rebated system and the proposed expansion. The existing system will be entered into the PSEGLI incentive Calculator and the dollar amount of such prior system will be subtracted from the maximum total rebate amount available for the customer.

Instructions to Process the Solar PV System Rebate Solar PV Rebate Processing documents can be downloaded from http://www.psegliny.com/Commercial/efficiency/renewables/solar.html and Power Asset Management documents can be downloaded from http://www.psegliny.com/company/papers/interconnect-SGIP.html . Incomplete applications and missing documentation will delay or prevent the processing of an application. Customers must use a copy of this form to submit any changes or updates to their solar PV System. 1

As part of PSEGLI’s rebated solar PV program submission process, all new Solar PV rebate application packets must be submitted concurrently to both Rebate Processing at [email protected] and Power Asset Management at [email protected]. Once the PV System is installed, submit a copy of the approved electric certificate issued by the Board of Fire Underwriters (or agency having jurisdiction) and Certificate of Completion to [email protected] and [email protected]. Upon receipt of this documentation and approval, PSEGLI will schedule and conduct a functional test of your PV System and install the appropriate meter. Itemized invoice(s) must contain all PV equipment model and serial numbers, and include purchase/installation date(s) and the PSEGLI Commercial Solar PV Program Application number. If any information is missing from the product invoice(s), PSEGLI will not be able to determine the Customer's eligibility for a Rebate. PSEGLI provides Rebates based in part on the information documented in the product invoice. Upon completing the installation of the PV System, send copies of your itemized PV invoices to [email protected]. After completion of all the steps listed above, please allow up to 60 days for delivery of the rebate check. Successful completion of an operational test of your installed PV System and submission of all documentation is required before the Rebate check will be issued. Page
