Abstract Background & objectives: Main constituents of cardiovascular diseases are: coronary artery disease (CAD), hypertension (H1N), type 2 diabetes mellitus (T2DM) and atherosclerotic cerebrovascular accident (CVA). ' Endothelial dysfunction may be the pathophysiological link between hypertension, T2DM, CAD and CVA. It is possible that endothelial dysfunction gets accelerated due to environmental and hereditary factors in a subset of CAD. Therefore, in the present study, we have attempted to prove that essential hypertension, T2DM, CAD and atherosclerotic CVA are continuum of same clinical spectrum in a subset of CAD. Methods:Thestudy was conductedin Department ofMedicine/PreventiveCardiologyfromJan 2007toDec. 2007. A total of 625 consecutive cases of CAD were studied. They were divided into two groups- group 1 (CAD alone) and group 2 (those with all three vascular disorders Le. CAD, H1N, T2DM and/ or thrombotic CVA). The temporal development of CAD, T2DM, HTN and/or CVA in each case was noted. Details of age, sex, smoking and family history ofT2DM, hypertension, CAD and CVA was taken in the first degree relatives of each patient. Results: There were 197 cases (31.5%)in group 1 and 117 cases (18.7%)in group 2. Mean age in group 1 was 54.94:t13.6 years, while in group 2 was 60.6:t1O.2years (p
