Cerebrovascular Ultrasound in Stroke Prevention and Treatment, 2nd edition Edited by Andrei V Alexandrov New York, USA: Wiley-Blackwell, 2011 ISBN: 978-1405195768, Price: £129.99 Cerebrovascular Ultrasound in Stroke Prevention and Treatment is a welcome addition to the rather sparse offering of textbooks specializing in neurosonology and cerebral haemodynamics. The pages are lavishly illustrated, and encourage a holistic approach to diagnosing cerebrovascular disorders, bringing together an understanding of cerebrovascular anatomy, blood-flow physiology and the fundamental principles of ultrasound through numerous examples and interesting case studies. In addition to presenting ‘textbook’ examples of Doppler spectra, the authors also provide suboptimal examples to illustrate pitfalls and give tips for improvement. The chapters follow a natural progression, from the basic principles of ultrasound, fundamental haemodynamic concepts and ultrasound waveform interpretation criteria, to the use of ultrasound in patient management and the discussion of clinical cases. The authors present a framework for structured assessment of extracranial and intracranial vasculature, with emphasis on the need for standardization. Principles and procedures for performing transcranial Doppler (TCD), power motion Doppler (PMD) and transcranial colour duplex (TCCD) imaging are presented alongside their respective advantages and disadvantages. All steps are

clearly defined, and practical ‘insider’ tips for improving accuracy are suggested. Of particular interest is the focus on spectral waveform analysis in Chapter 5. The last few sections of the book cover diagnostic criteria, differential diagnosis and treatment options. The authors also discuss briefly the potential use of ultrasound as a therapeutic tool. Chapters are contributed by an international and multidisciplinary team of authors. Each chapter is well written, of a consistently high standard, and is richly illustrated with figures and examples. As with many textbooks featuring a number of contributors, there are inevitable overlaps in the topics covered. The section on haemodynamics is long compared with other sections, and was difficult to follow at times. What makes this book unique is its practical emphasis and focus on waveform recognition as an assessment tool. The authors present an interesting variety of clinical cases, and this helps the reader to digest and consolidate more abstract material in earlier sections. The main focus of this book is the practical use of ultrasound to improve the diagnostic process and management of patients with cerebrovascular disorders. Despite the hefty price-tag, this book would most certainly be suitable for anyone with a special interest in cerebrovascular ultrasound, including physicians, sonographers, physicists and researchers.
